Three of the four rotator cuff muscles are used to draw the arm back and rotate the arm laterally and externally, while the fourth rotator cuff muscle is used to rotate the arm forward. The subscapularis muscle is responsible for medial and forward rotation.

This muscle collaborates with the deltoids and pectorals to propel the arm forward. This indicates that it is possible, but unlikely, to sustain an injury in this region without also injuring the pectoral or deltoid muscles. Irradiation means that when you contract a muscle, the surrounding muscles, particularly those of the antagonist, will also contract.

This means that you may experience discomfort when extending your arm forward. There is a high chance that you have damaged your rotator cuff in some way if you experience pain when pushing your arm forward and also when moving it back or in any direction.
